Ara-290
Also known as Cibinetide, ARA 290, pHBSP
ARA-290 (cibinetide) is an 11-amino-acid peptide modeled on a surface region of erythropoietin. It is designed to engage tissue-protective signaling without stimulating red blood cell production, and is studied in neuropathy and metabolic disease.

Overview
Erythropoietin protects tissues from injury through a receptor complex distinct from the one that drives red cell production. Researchers led by Michael Brines and Anthony Cerami identified helix B of erythropoietin as a key region and derived a short peptide from its aqueous face; its pyroglutamate form, ARA-290, does not stimulate erythropoiesis. Preclinical work covered stroke, nerve trauma, renal ischemia and wound healing. Clinical trials have examined small fiber neuropathy in sarcoidosis, type 2 diabetes with neuropathy, and diabetic macular edema. ARA-290 is not an approved drug.
Class: Erythropoietin-derived innate repair receptor agonist
Mechanism of action
How the compound acts, as described in the literature.
Activates the innate repair receptor, a heteromer of the erythropoietin receptor and the β common receptor (CD131), rather than the erythropoietin receptor homodimer that drives erythropoiesis.
Preclinical studies report reduced pro-inflammatory signaling and less apoptosis in injured tissue, along with improved wound healing in diabetic mice.
In patients with sarcoidosis-associated small fiber neuropathy, it was associated with increases in corneal nerve fiber density and area measured by corneal confocal microscopy.
Rodent studies report effects on neuropathic pain pathways, including modulation of TRPV1 channel activity.

In vitro and rodent study
PMID 18676614Nonerythropoietic, tissue-protective peptides derived from the tertiary structure of erythropoietin.
Brines M, et al. · Proc Natl Acad Sci U S A
Peptides based on erythropoietin helix B, including an 11-amino-acid peptide from its aqueous face, were tissue-protective in models of stroke, retinal edema, nerve trauma, renal ischemia-reperfusion and wound healing without stimulating erythropoiesis.
2013
Placebo-controlled trial
PMID 24136731ARA 290 improves symptoms in patients with sarcoidosis-associated small nerve fiber loss and increases corneal nerve fiber density.
Dahan A, et al. · Mol Med
In a blinded, placebo-controlled trial in patients with sarcoidosis-associated small nerve fiber loss, ARA-290 was associated with improved neuropathic symptoms, increased corneal small nerve fiber density and greater 6-minute walk distance.
2017
Phase 2b RCT
PMID 28475703Cibinetide Improves Corneal Nerve Fiber Abundance in Patients With Sarcoidosis-Associated Small Nerve Fiber Loss and Neuropathic Pain.
Culver DA, et al. · Invest Ophthalmol Vis Sci
In 64 patients with sarcoidosis-associated small nerve fiber loss and neuropathic pain, one of three cibinetide dose groups showed a significant placebo-corrected increase in corneal nerve fiber area, and these changes correlated with 6-minute walk results.
Evidence & safety
What the research does not show, and the adverse events and regulatory status it reports.
Clinical trials have been small and short, and most were conducted by researchers affiliated with the developer, Araim Pharmaceuticals. In the phase 2b sarcoidosis trial only one of three dose groups met the primary endpoint, and a nine-patient phase 2 diabetic macular edema trial showed no improvement in visual acuity or retinal thickness, although it reported no serious adverse events or anti-drug antibodies. Cibinetide is investigational and not approved.
