Cagrilintide
Also known as NN9838, NNC0174-0833, AM833
Cagrilintide is an investigational long-acting analogue of amylin, a pancreatic hormone involved in satiety. Researchers study it alone and with semaglutide to examine amylin receptor pathways in body-weight regulation.

Overview
Cagrilintide was developed by Novo Nordisk. Native amylin is co-secreted with insulin by pancreatic beta cells, but it is short-lived and tends to form amyloid fibrils, which complicates drug development. Cagrilintide is a stabilized, lipidated amylin analogue designed to resist aggregation and to circulate for days. It activates amylin receptors and the calcitonin receptor, and is therefore described as a dual amylin and calcitonin receptor agonist. It has been tested alone in a phase 2 obesity trial and is being developed mainly in combination with semaglutide.
Class: Long-acting amylin analogue (amylin and calcitonin receptor agonist)
Mechanism of action
How the compound acts, as described in the literature.
Amylin receptors are complexes of the calcitonin receptor with receptor activity-modifying proteins (RAMP1, RAMP2, or RAMP3). Cagrilintide activates these amylin receptor subtypes and the calcitonin receptor, and structural studies show an amylin-like binding mode.
Its main site of action is the hindbrain, where amylin receptors in the area postrema and nucleus of the solitary tract regulate satiety and meal size.
In rats, long-term cagrilintide increased prolactin-releasing hormone (Prlh) expression in Calcr-expressing hindbrain neurons, and knocking down Prlh blocked its effects on energy balance.
Native amylin also slows gastric emptying and suppresses glucagon after meals. These actions are thought to complement GLP-1 receptor agonism.

2021
Phase 2 RCT
PMID 34798060Once-weekly cagrilintide for weight management in people with overweight and obesity: a multicentre, randomised, double-blind, placebo-controlled and active-controlled, dose-finding phase 2 trial.
Lau DCW, et al. · Lancet
In 706 adults with overweight or obesity without diabetes, 26 weeks of cagrilintide produced dose-dependent mean weight reductions of 6.0% to 10.8%, versus 9.0% with liraglutide and 3.0% with placebo.
2021
Medicinal chemistry study
PMID 34288673Development of Cagrilintide, a Long-Acting Amylin Analogue.
Kruse T, et al. · J Med Chem
Described the structure-activity work behind cagrilintide, a stable, lipidated amylin analogue designed to overcome native amylin's short half-life and tendency to form amyloid fibrils, and its selection for obesity trials.
2026
Rodent and primate study
PMID 42260119A cross-species atlas of the dorsal vagal complex reveals neural mediators of the effects of cagrilintide on energy balance.
Ludwig MQ, et al. · Nat Metab
Mapped brainstem cell types in rat, mouse, and macaque. Cagrilintide acted on Calcr-expressing neurons, and knocking down Prlh in the dorsal vagal complex blocked its energy-balance effects in rats but not those of semaglutide.
Evidence & safety
What the research does not show, and the adverse events and regulatory status it reports.
Cagrilintide is investigational and not approved on its own. Evidence for cagrilintide alone rests mainly on one 26-week phase 2 trial, in which gastrointestinal events, mainly nausea, were more common than with placebo (nausea 20% to 47% versus 18%) and 10% of participants permanently discontinued treatment, mostly because of adverse events. Most studies were funded by its developer.
