Cagrilintide 5mg + Semaglutide 5mg
Also known as CagriSema
This item pairs cagrilintide, a long-acting amylin analogue, with semaglutide, a GLP-1 receptor agonist. The same pairing, known as CagriSema, has been studied in phase 3 trials in obesity and type 2 diabetes.

Overview
CagriSema is the name used in Novo Nordisk's clinical program for the co-administration of cagrilintide and semaglutide, two separately developed peptides. The rationale is that amylin and GLP-1 act through distinct receptors and partly distinct brain pathways, so combined activation may reduce food intake more than either alone. Phase 1 and phase 2 studies were followed by the REDEFINE phase 3 program in adults with overweight or obesity, with and without type 2 diabetes. A cardiovascular outcomes trial, REDEFINE 3, is under way. Published evidence comes from this sponsor-run program.
Class: Amylin analogue and GLP-1 receptor agonist combination
Mechanism of action
How the compound acts, as described in the literature.
Cagrilintide activates amylin receptors (calcitonin receptor and RAMP complexes) and the calcitonin receptor, acting mainly in hindbrain regions that regulate satiety and meal size.
Semaglutide activates the GLP-1 receptor, increasing glucose-dependent insulin secretion, lowering glucagon, slowing gastric emptying, and reducing appetite through hypothalamic and hindbrain pathways.
In rats, knocking down hindbrain Prlh blocked the energy-balance effects of cagrilintide but not those of semaglutide, supporting partly separate circuits for the two components.
In a phase 1b study, cagrilintide did not alter semaglutide exposure or elimination. Both components had half-lives of roughly six to eight days.

2025
Phase 3 RCT
PMID 40544433Coadministered Cagrilintide and Semaglutide in Adults with Overweight or Obesity.
Garvey WT, et al. · N Engl J Med
REDEFINE 1 randomized 3,417 adults with overweight or obesity without diabetes for 68 weeks. Mean body-weight change was -20.4% with the combination versus -3.0% with placebo; monotherapy arms were also included.
2025
Phase 3 RCT
PMID 40544432Cagrilintide-Semaglutide in Adults with Overweight or Obesity and Type 2 Diabetes.
Davies MJ, et al. · N Engl J Med
REDEFINE 2 randomized 1,206 adults with type 2 diabetes and overweight or obesity. At 68 weeks, mean body-weight change was -13.7% versus -3.4% with placebo; 73.5% versus 15.9% reached HbA1c of 6.5% or lower.
2021
Phase 1b RCT
PMID 33894838Safety, tolerability, pharmacokinetics, and pharmacodynamics of concomitant administration of multiple doses of cagrilintide with semaglutide 2·4 mg for weight management: a randomised, controlled, phase 1b trial.
Enebo LB, et al. · Lancet
In 95 treated adults with overweight or obesity, adding escalating cagrilintide doses to semaglutide was generally well tolerated, did not alter semaglutide pharmacokinetics, and produced greater 20-week weight loss than semaglutide plus placebo at higher cagrilintide doses.
Evidence & safety
What the research does not show, and the adverse events and regulatory status it reports.
Gastrointestinal adverse events, including nausea, vomiting, diarrhea, constipation, and abdominal pain, affected 79.6% of combination participants versus 39.9% with placebo in REDEFINE 1, and 72.5% versus 34.4% in REDEFINE 2; most were transient and mild to moderate. The combination is investigational, with a US marketing application filed in December 2025. All trial evidence comes from Novo Nordisk's own investigational formulations, and no study has examined other preparations of the two peptides.
