Cerebrolysin
Also known as FPF-1070
A preparation of low-molecular-weight peptides and free amino acids derived from porcine brain. It is studied for neurotrophic-like activity and has been tested in randomized trials in stroke and dementia.

Overview
Cerebrolysin is produced by enzymatic breakdown of purified porcine brain proteins, yielding a mixture of low-molecular-weight peptides and free amino acids. It is approved for neurological indications in a number of countries in Europe and Asia but not in the United States. Preclinical studies describe neurotrophic-like effects on neuronal survival and neurogenesis. Clinical research includes randomized trials in acute ischemic stroke, post-stroke motor recovery, and dementia, with mixed results. Because it is a multicomponent biological preparation, its effects cannot be attributed to a single defined molecule or receptor.
Class: Porcine brain-derived peptide preparation
Mechanism of action
How the compound acts, as described in the literature.
Contains a mixture of short peptides and amino acids; preclinical studies describe neurotrophic factor-like effects on cultured neurons.
In rats after embolic stroke, it increased neural progenitor proliferation, neurogenesis, oligodendrogenesis, and axonal remodeling; blocking Sonic hedgehog signaling with cyclopamine abolished these effects (Zhang 2013).
Animal studies have also examined effects on oxidative stress and apoptosis-related markers; which components drive specific effects has not been established.
As a multicomponent preparation, it lacks a single defined receptor target, which complicates mechanistic interpretation.

2012
Phase 4 RCT
PMID 22282884Cerebrolysin in patients with acute ischemic stroke in Asia: results of a double-blind, placebo-controlled randomized trial.
Heiss WD, et al. · Stroke
In 1,070 patients with acute ischemic stroke (CASTA), Cerebrolysin did not differ from placebo on the combined primary endpoint at 90 days. A post hoc analysis showed a trend favoring Cerebrolysin in severe stroke.
2016
Phase 2 RCT
PMID 26564102Cerebrolysin and Recovery After Stroke (CARS): A Randomized, Placebo-Controlled, Double-Blind, Multicenter Trial.
Muresanu DF, et al. · Stroke
Patients receiving Cerebrolysin alongside standardized rehabilitation showed better upper-limb motor function (Action Research Arm Test) at day 90 than placebo. Authors called the study exploratory and called for a larger confirmatory trial.
2023
Cochrane systematic review
PMID 37818733Cerebrolysin for acute ischaemic stroke.
Ziganshina LE, et al. · Cochrane Database Syst Rev
Across seven RCTs (1,773 participants), Cerebrolysin or Cortexin probably made little or no difference to all-cause death in acute ischemic stroke. Moderate-certainty evidence suggested a possible increase in non-fatal serious adverse events.
Evidence & safety
What the research does not show, and the adverse events and regulatory status it reports.
The largest acute stroke trial (CASTA) missed its primary endpoint, and the 2023 Cochrane review found no effect on mortality, no data on functional outcome, and moderate-certainty evidence of more non-fatal serious adverse events. Positive motor-recovery results come from smaller trials that their authors described as exploratory. Cerebrolysin is approved in several countries in Europe and Asia but not in the United States.
