Survodutide
Also known as BI 456906
Survodutide is an investigational peptide that activates both the glucagon and GLP-1 receptors. Researchers study it in obesity and in metabolic dysfunction-associated steatohepatitis (MASH), where hepatic glucagon signaling is of particular interest.

Overview
Survodutide was developed by Boehringer Ingelheim, with Zealand Pharma, under the code BI 456906. Its design draws on the dual activity of oxyntomodulin, a gut hormone that activates both the glucagon and GLP-1 receptors. The peptide carries a C18 fatty acid side chain that extends its half-life. After phase 2 trials in obesity and in MASH with fibrosis, the phase 3 SYNCHRONIZE program has reported results in obesity with and without type 2 diabetes and in steatotic liver disease. It is not an approved drug.
Class: Dual glucagon and GLP-1 receptor agonist
Mechanism of action
How the compound acts, as described in the literature.
Activates the GLP-1 receptor, increasing glucose-dependent insulin secretion, slowing gastric emptying, and reducing food intake. Studies in GLP-1 receptor knockout mice were used to confirm this component.
Activates the glucagon receptor, which is highly expressed in the liver. In mice, engagement was shown through liver NNMT expression and changes in circulating amino acids and FGF21.
In mice it reduced body weight more than maximally effective semaglutide doses, an effect attributed to increased energy expenditure in addition to lower food intake.
Hepatic glucagon receptor signaling promotes fat oxidation, which is the rationale for studying it in MASH, where a phase 2 trial reported histologic improvement.

2026
Phase 3 RCT
PMID 42253238Survodutide Once Weekly for the Treatment of Adults with Obesity.
le Roux CW, et al. · N Engl J Med
SYNCHRONIZE-1 randomized 725 adults with obesity without diabetes. At 76 weeks, mean body-weight change was -12.2% and -13.0% at two dose levels versus -5.4% with placebo (treatment-regimen estimand).
2024
Phase 2 RCT
PMID 38847460A Phase 2 Randomized Trial of Survodutide in MASH and Fibrosis.
Sanyal AJ, et al. · N Engl J Med
In 293 adults with biopsy-confirmed MASH and stage F1 to F3 fibrosis, MASH improvement without worsening fibrosis at 48 weeks occurred in 43% to 62% across dose levels versus 14% with placebo.
2022
Rodent and in vitro study
PMID 36356832BI 456906: Discovery and preclinical pharmacology of a novel GCGR/GLP-1R dual agonist with robust anti-obesity efficacy.
Zimmermann T, et al. · Mol Metab
Characterized BI 456906 in cell assays and mice. It reduced body weight more than maximally effective semaglutide doses through higher energy expenditure and lower food intake, with engagement of both receptors confirmed in knockout and reporter mice.
Evidence & safety
What the research does not show, and the adverse events and regulatory status it reports.
Survodutide is investigational and not approved. Gastrointestinal events occurred in 80.9% and 89.7% of the two survodutide groups versus 47.9% with placebo in SYNCHRONIZE-1. In the phase 2 MASH trial, nausea (66% versus 23%), diarrhea (49% versus 23%), and vomiting (41% versus 4%) were more frequent than with placebo, and serious adverse events occurred in 8% versus 7%.
