Overview
Ipamorelin is a pentapeptide growth hormone secretagogue developed in the 1990s. It acts on the ghrelin receptor rather than the GHRH receptor and, in animal studies, released growth hormone without the pronounced ACTH, cortisol or prolactin rises seen with earlier GH-releasing peptides. Human pharmacokinetic work characterized its short half-life. It was later tested in a phase 2 trial for postoperative ileus, drawing on the ghrelin receptor's role in gut motility, where it did not meet its efficacy endpoints.

Ghrelin receptor activity in the gastrointestinal tract has prokinetic effects, the basis for its investigation in postoperative ileus.

In 114 bowel-resection patients, ipamorelin was well tolerated, but time to first tolerated meal (25.3 vs 32.6 h) did not differ significantly from placebo (p = 0.15).
