Overview
GHRP-2, also known as pralmorelin, is one of the synthetic GH-releasing peptides that came out of C.Y. Bowers' work, predating the discovery of ghrelin. It activates the ghrelin receptor and produces strong, dose-related growth hormone release. In Japan it is approved as a single-administration diagnostic test for adult GH deficiency. Human studies also show that, like ghrelin, it increases appetite and food intake. It can modestly raise ACTH, cortisol and prolactin.
Class: Ghrelin receptor (GHS-R1a) agonist
Mechanism of action
How the compound acts, as described in the literature.
Agonist at the ghrelin receptor (GHS-R1a) in the pituitary and hypothalamus.
Stimulates GH release directly at somatotrophs and indirectly by enhancing hypothalamic GHRH and opposing somatostatin.
Ghrelin receptor activation in appetite centers increases hunger and meal size.
Less selective than ipamorelin: increases in ACTH, cortisol and prolactin are reported.

2007
Clinical diagnostic study
PMID 17609397A simple diagnostic test using GH-releasing peptide-2 in adult GH deficiency.
Chihara K, et al. · Eur J Endocrinol
Evaluated a GHRP-2 stimulation test as a simple diagnostic for adult GH deficiency, supporting its use as a diagnostic agent.
2005
Human crossover study
PMID 15699539Growth hormone releasing peptide-2 (GHRP-2), like ghrelin, increases food intake in healthy men.
Laferrère B, et al. · J Clin Endocrinol Metab
In 7 lean, healthy men, GHRP-2 infusion increased food intake at a buffet meal by about 36% versus saline, with every participant eating more.
2004
Human study
PMID 15126555Sustained elevation of pulsatile growth hormone (GH) secretion and insulin-like growth factor I (IGF-I), IGF-binding protein-3 (IGFBP-3), and IGFBP-5 concentrations during 30-day continuous subcutaneous infusion of GH-releasing peptide-2 in older men and women.
Bowers CY, et al. · J Clin Endocrinol Metab
In older men and women, 30 days of continuous GHRP-2 infusion sustained elevated pulsatile GH secretion and raised IGF-I, IGFBP-3 and IGFBP-5.
Evidence & safety
What the research does not show, and the adverse events and regulatory status it reports.
Its only approval (Japan) is as a single-use diagnostic, not for repeated administration. It increases appetite and can raise cortisol and prolactin. Long-term human safety data are lacking.
