Overview
Tesamorelin is a synthetic analogue of all 44 amino acids of human GHRH, with a trans-3-hexenoyl group added at the N-terminus to resist enzymatic degradation. It stimulates endogenous growth hormone secretion and raises IGF-1. Phase 3 trials in people with HIV and abdominal fat accumulation led to US approval in 2010 for that single indication. Later randomized studies examined its effect on liver fat and non-alcoholic fatty liver disease in the same population.
Class: Stabilized GHRH analogue
Mechanism of action
How the compound acts, as described in the literature.
Agonist at the pituitary GHRH receptor, stimulating pulsatile growth hormone secretion.
The N-terminal trans-3-hexenoyl modification protects against dipeptidyl peptidase-4 cleavage, extending activity.
Raised GH and IGF-1 increase lipolysis, with clinical trials showing preferential reduction of visceral rather than subcutaneous fat.
Studies also report reductions in liver fat, attributed to GH-mediated changes in hepatic lipid handling.

2007
Phase 3 RCT
PMID 18057338Metabolic effects of a growth hormone-releasing factor in patients with HIV.
Falutz J, et al. · N Engl J Med
In 412 patients with HIV and abdominal fat accumulation, visceral fat fell 15.2% with tesamorelin over 26 weeks versus a 5.0% rise with placebo; IGF-I rose 81%. More tesamorelin patients withdrew because of adverse events.
2014
Randomized controlled trial
PMID 25038357Effect of tesamorelin on visceral fat and liver fat in HIV-infected patients with abdominal fat accumulation: a randomized clinical trial.
Stanley TL, et al. · JAMA
Over 6 months in 50 patients, tesamorelin reduced visceral fat and liver fat versus placebo. Fasting glucose rose at 2 weeks, but the change was not significant at 6 months.
2019
Randomized controlled trial
PMID 31611038Effects of tesamorelin on non-alcoholic fatty liver disease in HIV: a randomised, double-blind, multicentre trial.
Stanley TL, et al. · Lancet HIV
In people with HIV and fatty liver disease, tesamorelin reduced hepatic fat fraction compared with placebo over one year.
Evidence & safety
What the research does not show, and the adverse events and regulatory status it reports.
It is approved in the US only for excess abdominal fat in HIV-associated lipodystrophy. Trials reported more adverse-event withdrawals than placebo and early rises in fasting glucose, and visceral fat reaccumulated after treatment stopped. Evidence outside HIV populations is limited.
